Please submit enrollment 2 weeks in advance of your effective date.
New employees are eligible after their waiting period. At their own expense new employees should keep prior coverage during
the waiting period or consider Short Term Health Insurance opens new window or other options.
Open Enrollment allows employees and dependents to enroll in the medical plan without underwriting or penalty
if they have previously waived coverage.
For Qualifying Events you have 30 days to make enrollment changes from;
- 1; Marriage, divorce, or legal separation
- 2; birth or adoption (effective on date of birth or adoption)
- 3; death of a covered dependent
- 4; loss or gain of eligibility for insurance coverage for you or a covered dependent (note * coverage must not be a student or individual policy)
- 5; change in spouse’s employment status
- 6; change in health insurance eligibility due to a relocation of residence or work place
Terminated employee coverage goes to the end of the month of the day last worked.
After that the former employees options to consider at their own expense include;
- New employer group health program - there may be a waiting period.
- Short Term Health Insurance opens new window.
- Individual / Family Insurance opens new window.
- COBRA continued group coverage. For employer groups of more than 20 employees in the prior year.
- HIPAA Guarantee Issue - may be best for people with serious health conditions.
